Client Information & Client Consent
This document is information in regards to the client and the student. It also includes consent for Advocacy Partners to bill and acknowledges that the client will pay invoices promptly.
Service Agreement - Phone Consultation
This document is an agreement between the client and Advocacy Partners for the phone consultation.
Service Agreement - IEP/504 Review
This document is an agreement between the client and Advocacy Partners for the an IEP/504 Review. This agreement outlines both Advocacy Partners support and the clients expectation for payment.
Service Agreement - IEP/504 Meeting
This document is an agreement between the client and Advocacy Partners for the an IEP/504 Meeting. This agreement outlines both Advocacy Partners support and the clients expectation for payment.
Service Agreement - Other/Miscellaneous Support
This document is an agreement between the client and Advocacy Partners for additional support needed that is not outlined in the above agreements. This agreement is per the discretion of Advocacy Partners and their ability to support.
